1)Applicants may be adults or children 12 years and up (with consent of parent/ legal guardian).
2)Must be up to date with basic dental care (within the past 12 months).
3)Priority given to existing clients of Bower Dental & Southpointe Dental, Red Deer.
4)Applicants selected will be notified by phone/mail and be required to be personally interviewed.
5)All applications become the property of the sponsors and will not be returned.
6)No phone calls will be accepted.
7)Applicants
should submit a one page letter describing what they would like done to
have the "Smile of their Dreams", and include why they should be
chosen. Be sure to include a recent photo (which will become the
property of the sponsor).
8)Judges reserve the right to decide all
features of the awards offered, while the applicant has the right to
refuse any or all of the procedures presented. All
procedures are very safe but there may be a small incidence of
complications which will be discussed.
